2011 Summer Park Program

 

To:  The Parents of Ottoville School District 2011 Summer Park Program Participants, all students who participate in the Summer program must:  1) register 2) pay a $30.00 fee and 3) parent/guardian must sign this form.  Please complete the attached form and send to P.O Box 332 Ottoville, OH 45876 by March 11, 2011. After March 11, 2011, a $10/child late fee will be charged.  All checks should be made payable to The Ottoville Park Board.  (Please send no money to school.)

 

 

Boys Traveling                       To be eligible, a player must be at least 9 before August 1, and no

Little League                           more than 12 before August 1, or no older than present 6th grade.  If you are not chosen to play on the Traveling Little League, you are eligible to play Ottoville City League.  (Shirts included in fee; the coaches will obtain sizes. Players will be responsible for the cost of accessories.)

                

Ottoville City                           To be eligible, a player must be presently in the second grade and no

League                                     more than 12 years old by August 1.

                                                 (Cost of t-shirts included in fee.  Please indicate child’s size on attached registration form.)

 

Pony League                           To be eligible you must be currently in the 7th or 8th grade. (Shirts included in fee; the coaches will obtain sizes.  Players will be responsible for the cost of accessories.)

 

Acme HS                                 To be eligible, a boy must presently be in the 9th, 10th, or 11th

Baseball                                   (Games played on HS diamond)

 

Traveling Girls                        To be eligible, a girl must presently be in the 9th, 10th, or 11th grade.

 Softball (A)                             (Games usually played Tuesday and Thursday evenings and on HS diamond)

 

Traveling Girls                        To be eligible, a girl must presently be in the 7th or 8th grade.

 Softball (B)                             (Cost of t-shirts included in fee.  Please indicate child’s size on attached registration form.) (Games usually Tuesday and Thursday evenings)

 

Traveling Girls                        To be eligible a girl must be presently in the 5th or 6th grade.

 Softball (C)                             (Cost of t-shirts included in fee.  Please indicate child’s size on attached registration form.) (Games usually Monday and Wednesday evenings)

 

Girls Sugar and                      To be eligible a girl must be presently in the 2nd, 3rd, or 4th grade.

 Spice                                       (Cost of t-shirts included in fee.  Please indicate child’s size on attached registration form.) (Games played Tuesday and Thursday evening)

 

T-Ball                                       To be eligible girls and boys must be presently pre-K (must attend Kindergarten in the fall), K or 1st grade. (Cost of t-shirts included in fee.  Please indicate child’s size on attached registration form.) (Games played Monday and Wednesday evening)

 

On the attached sheet, please list the names, age, grade, and appropriate program you would like to have your child, or children participate in.  Please fill in all information, sign, and return with registration fee.  If applicable please mark shirt size.  Your money will be refunded if you child chooses not to play before the season starts or before the T-shirts are ordered.  Thank You.  Todd Knippen, Park Board President.
